Enrollment

Our application process is designed to see our program in action, meet our staff, and discuss the goals of each prospective client.

 

A 6 hr day trial period is suggested before enrollment into the center.​

To begin the application process:​

1. Interview with prospective clients, parents/guardians, and the Beyond Care Adult Day Admissions team.

2. Complete the Client application form, save as a PDF file and email to Info@beyondcareadultday.com..

3. Please return the application and payment to:
